Chronic Disease Management
A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions.
Our Clinical Nurse Consultants – Chronic Disease Management, Sally Lowe and Carolyn Thatcher, help clients to enjoy the best quality of life while living with an ongoing disease, such as fibromyalgia, diabetes, lung disease, heart disease, arthritis,
chronic pain, chronic fatigue or certain mental health conditions.
Clients are often referred to Carolyn and Sally by a General Practitioner, Registered Nurse or an Allied Health Professional.
Clients can also self-refer by contacting BCH’s Access team: Access@basscoasthealth.org.au or phoning 5671 3175.
Sally and Carolyn’s services are provided to people aged under 65 (under 50 years for Aboriginal and Torres Strait Islanders) through the Home and Community Care Program for Younger People. We also provide their services to people aged over 65.
However, the Chronic Disease Management items are designed for patients who require a structured approach and to enable doctors to plan and coordinate the care of patients with complex conditions, requiring ongoing care from a multi-disciplinary team.
