Health professionals can access our Referrals
Social Support Groups assist people’s ability to remain connected to the community by providing a range of enjoyable and meaningful activities that build upon independence. Participants are picked up and dropped off from home / place of residence.
An extension of these programs is the Weekend Outreach Program, which offers eligible residents within the Bass Coast Shire, fully supported weekends in suitably selected holiday accommodation.

Schedule of Rates for Services Provided by Bass Coast Health
Note: Costs applicable from 01 July 2024 - 30 June 2025. Rates will then be reviewed and adjusted accordingly, as per the required fee schedule guidelines from the Victorian Department of Health and Human Services.
