What we do

Bass Coast Health’s ‘Armitage House’ is a Sub-Acute Inpatient service consisting of 24 beds that provide Geriatric Evaluation and Management, Rehabilitation, Palliative Care and Stroke Rehabilitation.

The Geriatric Evaluation and Management (GEM) program provides specialist assessment and management by a multidisciplinary team comprising medical, nursing and allied health staff. GEM refers to care of chronic or complex conditions associated with aging, cognitive dysfunction, chronic illness or disability. 

Rehabilitation is proactive, goal orientated and time-limited care that targets clients with loss of function or ability from any cause (congenital or acquired). The aim is to improve and/or prevent deterioration of function to achieve the highest possible level of independence, physical, psychological and social wellbeing.

Palliative care is an approach that improves the quality of life of patients and their families facing life-threatening illness, through the prevention and relief of suffering by means of early identification, assessment and treatment of physical issues such as pain, along with psychosocial and spiritual concerns. 

For some people coming into palliative care can be confronting physically, spiritually and mentally. Our team can educate and support the consumer and their families through their admissions and discharge. 

Patients who have been diagnosed with a stroke or TIA and require rehabilitation and/or discharge planning to transition to home or other discharge destinations may be accepted for admission to Armitage House.

Patients admitted to Armitage House receive person-centred care focused on enhancing their health, wellbeing, care experience, and achieving the best possible outcomes tailored to their individual needs. Our focus is to collaborate with patients, families and carers to promote person-centred care, ensuring consumers feel safe and empowered to discuss all aspects of their health care journey and discharge with their clinicians.

Admission to Armitage may be via a referral from your GP or referral to the Geriatrician from another health service, using this referral form: 


Will I have to pay for services?

What should I bring if I am staying in hospital?

•    Your Medicare card
•    Your Healthcare card, Pension Card, Department of Veterans’ Affairs card (if you have one)
•    Adverse drug alert card (if you have one)
•    An up-to-date list of your medications
•    Any relevant test results or scans, such as x-ray films, scans, ultrasounds or any other test results you have which are related to your condition 
•    Any aids that you would usually use on a daily basis (e.g., glasses, hearing aid, walking frame)
•    Your general practitioner (GP) name, clinic and address (if you have one)
•    Your Advanced Care Directive (if you have one)
•    Your Transport Accident Commission (TAC) or WorkCover claim numbers (if you have one).
•    Your toiletries
•    Suitable clothing to wear during the day and overnight, including appropriate footwear to participate in rehabilitation.
•    Your mobile phone and charger (if you have one).

What we can expect from you

Acts of violence, threats, or verbal aggression towards staff members, other patients or visitors will not be tolerated.  Security or police will be immediately called to maintain safety for all.
We encourage all visitors to Bass Coast Health to report any concerning behaviours to our staff immediately, so that we can continue to provide care in a safe environment.
