Health professionals can access our Referrals
Social Workers help support people to access services to address the barriers that may present after an illness, injury or major life change.
Our Social Workers are able to provide:
• Psychosocial assessment in order to understand your individual needs
• Emotional support and advocacy
• Information and referrals for services available in the community
• Support with discharge planning for admitted patients.
Inpatient Services
Our inpatient social work team assists the person and their supports to ensure there is a smooth transition from hospital to their community. You can request to speak with a Social Worker during your admission or the multi-disciplinary team may also refer you to a Social Worker if additional support for discharge is identified.
Community
Social Workers can also support people in the community with linking to ongoing services and supports, such as aged care and NDIS. Social Workers are available at Wonthaggi Hospital and Phillip Island Community Hospital by appointment. In some circumstances, we also be able to complete a home visit.
Referrals
To refer to a community Social Worker, you can self-referral (use the form above) or contact our Access team by phoning 5671 3343. Referrals can be made by other services and agencies in the community.
For people over 65, we are able to accept referrals for Commonwealth Home Support Program (CHSP) through My Aged Care referral.
We are not able to accept referrals for children or young people under 18 years old. Please contact The Orange Door on 1800 319 354 to seek information about options for support.
