- Helping to identify Aboriginal patients
- Assisting patients navigate the hospital setting
- Providing information about Close the Gap
- Contacting family members and carers where required
- Supporting staff who are working with Aboriginal clients, through individual consultations
- Basic material aid: toiletries, emergency clothing, myki ticket for public transport
- Follow-up for outpatient appointments
| - Developing relationships with the Aboriginal community
- Ensuring BCH is culturally sensitive, safe and welcoming for Aboriginal people
- Liaising with other support workers who are supporting the patient
- Working with Aboriginal agencies for referrals and support services
- Providing advice about Aboriginal services a patient could be referred to
| - Providing the patient with information about services in the community and how they can access them on discharge
- Providing cultural safety training to staff
- Identifying and addressing barriers that may be preventing Aboriginal people from using BCH services
- Distribution of promotional material to community and within BCH
- Attending and participating in relevant committees, forums, seminars, training and meetings
- Implementing policies to improve services for Aboriginal patients
- Ensuring Aboriginal people have access to all of our services
